Liability
In today's world you can't be too careful about protecting yourself from frivolous lawsuits. It was for this reason that the EZ plans were taken off the market to begin with. I am only a homebuilder like yourself and I could not afford to provide my services under the constant threat of being sued. This is not because I don't trust my products. I am simply trying to protect myself from the sort of shotgun legal abuse that goes on these days. In order to prevent this sort of thing, I require all my customers to fill out and sign a liability release. If you would like to place an order, please download the liability release form below, print it out, fill it out, sign it, and mail it to me.

Now that the legal stuff is out of the way, click one of the order form links below. If you have Excel, click on the Excel order form and save it somewhere on your computer that you can find it again. Now, open it in Excel, fill it out and email it to me and print it out and include it with the liability release when you mail it. I would appreciate an electronic copy either way so I can keep it in my electronic records.

This order form calculates shipping charges for customers located in the continental United States. It also builds an invoice and will calculate Paypal fees if needed. Since it is virtually impossible to test every conceivable shipping area and combination of parts, the order form could be in error. I reserve the right to correct any charges if they are more than $10 in error. I'll try to let you know as soon as possible if this is the case. Having said that, this order form has consistently been very accurate. If you don't live in the continental U.S., just email me your address and I'll estimate the shipping for you. Also, if you don't see what you want to order on the form (i.e. elevators, Cozy III parts, SpaceShipOne feathers, etc) just email or call me and let me know what you're looking for.
To calculate Paypal fees, fill out the order page and then go to the invoice page. Select Paypal under payment type. The fees will be shown under "Additional Fees" and the new grand total will be highlighted in yellow.
If you don't have Excel, you can download the pdf version below. Just fill it out by hand and mail it to me. I can calculate the shipping for you when I receive it or you can call or email me and I'll figure it out then.

Payment
Payment can be made in the form of cash, personal check, cashier's check, money order, Paypal, or whatever else we can agree on. Make checks payable to Eureka CNC. Any fees associated with Paypal or any other form of payment will be passed on to the buyer. I will ship your items after the liability release is received, all payment is collected, and all checks have cleared.
If you want to pay by Paypal, contact me via email and I'll give you my Paypal account info. Any additional fees will be passed to the buyer. For this reason, a check usually works better.
All orders within the state of Texas or Nevada will be charged 8.25% sales tax on materials only. This includes all orders picked-up in person or delivered.
